The model ...

The weakest pupils are identified on entry into Year 7 - these pupils take the diagnostic test. It is used to assess attainment across each of the 11 strands of Mathematics. Targets are identified for 1 or more strands from this test and using the Diagnostic Grid.

A good strategy would be to stick the Diagnostic Grid into pupil exercise book so that a teaching assistant can easily identify which targets to work on. This way the pupil [and parents] also know what their targets are.

A focused teacher assessment can be carried out of a particular target and a new target established, again using the Diagnostic Grid to ensure progress across each strand of Mathematics.
